Permission To Skate Forms - Updated

*Those who have completed the registered form and had requested a PTS or an NRP , they are available for pick up in mailbox at 200 King Hiram St*

The Ontario Minor Hockey Association (OMHA) requires that every player trying out for a AAA team provide their AAA zone with a Permission to Skate (PTS) form which they must obtain from their home center, if trying out for an A/AA centre they require a non-resident passport(NRP) which they must obtain from their home center as well.

Please note that the Permission To Skate (PTS) form is not a player release nor is it an OHF AAA Waiver form.

To obtain a permission to skate form for AAA tryouts please register your child through the Hockey Canada Link below.
